In-Person Pick Up of $100 Vaccine Incentive

Thank you for getting vaccinated and helping to keep your community safe and healthy!

After getting the COVID-19 vaccine, you selected to pick up your $100 pre-paid debit card in person. If you changed your mind and want to receive your pre-paid debit card via email or have your card physically mailed to the mailing address you used to make your appointment, please go to www.vax4nyc.nyc.gov/incentives and select the "Virtual" or "Mailed" options.

You may only redeem one incentive. If you selected the virtual or mailed debit cards or any of the other incentives you are not eligible to receive the $100 debit card at the pickup location.

How to Pick Up Your $100 Incentive

To pick up your debit card, you will need to go to one of the locations listed below during their operating hours to find the Vaccine Incentive Pick Up Table. In order to redeem your $100 pre-paid debit card you will have to provide a form of photo identification, your Dose 1 Appointment #, Date of Birth, and Zip Code. The card will have funds on it within 48 hours of when you pick it up.
